Demonstration only. Reporting is switched off on purpose. A real safety pathway needs a validated workflow, clinician oversight, named response responsibilities, monitoring, and protocol-specific escalation before it can accept a single report. Until then, call your coordinator or (251) 455-6300.

What the live version would do

  1. Record what you reported, with the date and time.
  2. Route it to a named person on your study's research team, not a general inbox.
  3. Trigger clinician review within a defined timeframe set by the protocol.
  4. Escalate to the investigator and, where required, to the sponsor and the ethics board.
  5. Tell you what happened next.
